"Tom, have a shower and go to bed." sounds better to me.

Agreed, much better.

Which of these work?

A: Did you close the window? I told you to leave it open. It's stuffy in here.
B: But I didn't. It closed itself. I guess it's windy outside.

First and third (as modified above) work for me.

If someone, say, sings a song and asks me 'How did I do?', do I reply "very good" or "very well"?

And when a teacher praises a student for what they have said or done, does either work?
Very good, Tom.
Very well, Tom.

Here's my take on this: "very well" is less of a compliment than "very good".

Say there are some chickens on the road and I'm telling the driver these:

Watch the chickens!
Watch out for the chickens!
Mind the chickens!

Do all of them work?

Second and third, but not the first.

Do you use "head" as a verb like they do in the US? Would you use "to" or not?

Where are you going? = Where are you heading (to)? = Where are you headed (to)?

It's an 'age thing' I think people of my generation are more likely to say "Where are you going?" but the younger generation are my influenced by AE usage and would tend to say "Where are you heading to?" - note NOT "headed" AFAIK that's exclusively AE usage.
